To Register
for a Release to Play Out of State
(For the
2005/2006 Soccer Year*)
The
following procedure must be followed for your child to be Released
to Play Out of State:
- Before
you proceed please completely read these instructions.
- Click
on the “Registration & Release to Play Out of State” link
below.
- Enter
all information requested; if your child is registered with an
association for the 2005/2006* soccer year please contact the association
registrar for your child’s SRI ID number.
- Click
on “Submit Registration Information” at the bottom
of the application. A gray box will appear, read the contents.
- Click “OK”,
a new form complete with your child’s information is in the
next window.
- Print
two copies of this form, one is for your records, the second you
must sign and mail to Soccer Rhode Island along with the Out of
State Registration fee. To print the form, please clear all headers
and footers from your print setup and set all margins to .25”.
The
Out of State Registration fee for the “2004/2005 soccer year”*
is $15.00. The fee for the 2005/2006 soccer year* will be set on
August 29, 2005. You will be billed for any increase in the fee.
